Hospital at Home Group:

 

The program is designed to care for the less complex medical problems in the comfort of the patient’s home without compromising the safety, quality and standard of care.

The patients are selected according to the John Hopkins hospital at home criteria for admissions.


HOSPITAL AT HOME GROUP OBJECTIVES

a) Coordinate transfer of care.

b) Evaluate the patient by discussing with the transferring doctor, the appropriateness for hospital at home, based on the John Hopkins, Hospital at Home Program admission criteria

b) The physician discharging the patient from the ED or the office shall write a complete admitting order for the patient care.

c) Transfer the care to the hospital at home group.

d) The hospital at home group physician sees the patient within 24 hours and formally admit the patient to the program.

e) Daily visits shall be made by the RN or the LPN to administer the medications ordered by the attending physician.

f) The attending physician shall see the patient at least 2 times during the hospital at home. One at admission and the other at discharge. The attending physician may see the patient as often as deemed necessary.

f) They will appropriately transfer the care of the patient at the time of discharge to the receiving team via the EMR.


The hospital at home group also provide home care services for the home bound population. The patients with 2 or more admissions in the last year are eligible to be admitted to the hospital at home chronic care patient service and monitored weekly.
