THE SKILL NURSING HOME GROUP OBJECTIVES

The Skill Nursing Home Group accepts transfer from:

The Primary Care Physician Offices,

The Hospitalist group, either from the ED or at discharge 

The Hospital at Home Group.

We are responsible for the following:

a) Evaluate and admit the patient to the skilled nursing home according to set protocols.

b) Treat the patient daily at the skilled nursing home according to set guidelines and best practices.

c) Avoid the soft hospital admissions and readmission by treating the patients in the facility without compromising quality and patient safety.

d) Appropriately transfer the care of the patient at the time of discharge to the receiving team via the EMR

e) Provide medical director services for the nursing home if requested.

f) Provide wound care services with ongoing nursing education and coordination.

g) Coordinate all consultation services and manage the utilization of consultation services.

h) Manage Pharmacy Utilization.

i)  Provide nursing education support for our nursing home affiliates.

j)  Provide Respiratory services support, including nursing education and onsite tracheostomy management.
