Hospitalist Roles:
As an independent Hospitalist services, our roles are as follows:
a) Participate with the ER doctor in the decision making
process of whether to admit a patient or not via the ED based on Milliman and
Robinson or Interqual Criteria for acute inpatient hospitalization.
b) Should a decision be made, not to admit the patient, this will
be captioned as a ED discharge.
c) Actively Triage the patients;
to
the office via the mobile care management team,
to
home care via the Hospital at home group,
directly to the skilled nursing facilities via
the Skill Nursing Home group.
c) Admit patients according to set guidelines and best
practices.
d) Manage the patients while hospitalized.
e) Ensure an acceptable length of stay.
f) Manage the
appropriate utilization of consultants physicians based on acceptable
guidelines and best practices.
g) Efficiently utilize the hospital resources for
excellent patient outcome.
h) Transfer care at discharge via the electronic medical
record to the receiving provider at all time.
i) Follow up the result of any test ordered for the ER
discharged patient and address any abnormality thereof with the PCP or the
patient.
j) Available in our hospitals from 7 am to 12 midnight 7 days a week to actively participate in the care of the patient in the ER.
